All requests for Travel Permits, Team, and Player Status forms for a sanctioned tournament or event must be submitted to SAS using this form.

Team Status requests should be submitted together, by the Team Contact.  Any player additions to a previous request will be considered a NEW request.

Deadlines: 

This Form should be completed minimum six (6) business days before the earliest of the following dates (whichever comes first) to avoid any fees:

i)        SSA deadline

ii)       Event stated deadline

iii)     Event start date

Fees: 

• Team or Player Status requests received within 6 or more business days - $0

• Team Status requests received within 5 or less business days - $50

• Player Status requests received within 5 or less business days - $25 (applies per request, not per team)

*Names that are incomplete or misspelled will be disregarded.  SAS can only verify a player's status if the information provided is accurate.

Requests must include the fee, and a minimum of two (2) business days is required for processing. 

Requests submitted without sufficient time for processing may be subject to delays or denial.
